Definitions

Securing your information

All information collected is stored on a secure file server that follows generally accepted policies and procedures to protect all information. We use a cloud storage through ShareFile to store our data.

Personal identification information is stored on the server and only accessible to employees who need this information to perform a specific job. In some cases, personal identification information is stored on-site in our offices. This information is locked and accessible only to employees who need this information to perform a specific job.

Sensitive and private data exchanged between the website and you happens over a SSL secured communication channel and is encrypted and protected with digital signatures.

Transferring your data

Clinical SCORE does not sell or rent your personal identification information to others.

When requesting access, you may be asked to provide a reasonable level of identification to safeguard yours, and others, personal identification information from being accessed improperly.

Clinical SCORE will respond to the request within 30 days free of charge. Excessive requests, such as more than one request within 30 days, may require a reasonable fee.

Erasure

You have the right to delete or remove personal identification data from Clinical SCORE’s records.

Processing Preferences

You have the right to change or limit how your information is processed. Clinical SCORE will respond to the request within 30 days free of charge.

Data Portability

You have the option to obtain, copy, and transfer your personal data from Clinical SCORE.

Clinical SCORE will provide you with a ‘.xls’ or ‘.csv’ file format, with the requested data included, to you or a company you wish to transfer for your data to. We may be able to directly transfer data to a company if technically feasible, but we are not required to comply with the processing systems used by the other organizations.

When requesting access, you may be asked to provide a reasonable level of identification to safeguard yours, and others, personal identification from being accessed improperly. For this reason, we will not respond to companies requesting transfers without confirming with you directly.

We will respond to the request within 30 days free of charge. If a request is complex or requires more time, we may inform you that we have received your request within the 30 days and provide a timeline to provide you the information within two months.

Objection

At any point, you may opt out of receiving any communication from Clinical SCORE. This means you do not want to be contacted by Clinical SCORE for any reason. We will remove any of your personal identification information. Any previous agreements with you will be dropped to remove your information and we will no longer contact you.
