Non-personal identification information is information that is not traced back to any individual and is not used on its own to identify an individual.

Research activity or research activities are surveys, interviews or focus groups, or other research requests.

Clients are customers of Clinical SCORE who pay for research activities or services to be completed.

Information we collect

Clinical SCORE collects information from three main sources: visitors to the website, clients, and respondents.

To provide clear information of how visitor and client personal identification information is collected, please review the Website Privacy Policy here: http://clinicalscore.com/privacy-policy/

Respondent personal identification information may be collected by telephone, Internet survey or web survey, email, or through a list that has been provided by another market research firm, client, or other source.

Respondents may be asked to provide Clinical SCORE with the following information: first and last name, email address, phone number, mailing address, occupation, language preference, sensitive health information, etc. This information may be collected after a consenting process. If Clinical SCORE is collecting your information from you directly, we will ask you to provide this information voluntarily and you are not required to give us this information. However, it may prevent you from participating or responding to our research activities or for receiving payment.

If Clinical SCORE obtains your information through another market research firm, client, or a similar source, contracts will be in place to ensure that you have consented to have your information transferred to us.

How we use and disclose this information

To provide clear information of how visitor and client personal identification information is used and disclosed, please review the Website Privacy Policy here: http://clinicalscore.com/privacy-policy/

When participating in a research activity with us, your consent form will provide information with how your answers and personal identification information is used for the research.

We collect your answers to research activities to do market research or consulting work for our clients. These answers you provide are removed from most personal identification information you provide such as: name, phone number, email address, mailing address, country, language preference, etc. This information may be used for payment purposes, contacting you with further opportunities should you consent, providing research materials in your preferred language, etc. This personal identification information may be disclosed to a client should you consent to this transfer. The consent language will explain what information is being included, why, and to whom.

In some research, sensitive information is collected with your consent and will be removed from other personal identification information before being analyzed and combined with the answers of others. This sensitive personal identification information may include current medical conditions, diagnosis, health information, etc. This information may be disclosed to a client after being combined with the answers of others and presented in a report.

You may decide you do not wish to answer certain personal questions in the research activities you participate in and your consent will tell you that you are not required to answer or participate. If you decline to answer certain questions, it may impact access to some research activities.

If the research promises payment for your time, we will need the personal identification information (first and last name, phone number, email address, mailing address) when requested to provide you with the payment.

Clinical SCORE reserves the right to disclose personal identification information as required by law enforcement or for other legal purposes. While we do not anticipate sharing your information for these reasons, it could be required from time to time. Clinical SCORE aims to limit the types and amount of your information that we would be required to share.

Securing your information

All information collected is stored on a secure file server that follows generally accepted policies and procedures to protect all information. We use a cloud storage through ShareFile to store our data.

Personal identification information is stored on the server and only accessible to employees who need this information to perform a specific job. In some cases, personal identification information is stored on-site in our offices. This information is locked and accessible only to employees who need this information to perform a specific job.

Personal identification information required to provide payment is only accessible to those who will process and provide payment for a research activity.

Sensitive information that has been removed from other personal identification information (including name, mailing address, phone number, email, and country) will be accessible to team members at Clinical SCORE who are conducting the research.

Sensitive and private data exchanged between the website and you happens over a SSL secured communication channel and is encrypted and protected with digital signatures.

Clinical SCORE is in the United States. If you are not a U.S. resident, your information may need to be transferred to the United States. We may have a need to transfer your information to third-party services that are located outside of the United States to perform a specific job.

Third-party services we use are required to comply with our privacy policies before we share any data, personal or non-personal with them, and maintain at least the same level of privacy protection as required by law. We will work with you if you do not want your information to be shared with a third party.
